Welcome to the Athletics and Fine Arts Parent Portal!

We would like to thank you for taking part in our new process of accepting athletic and fine arts participation paperwork online. This new automated process will allow us to be more efficient in handling each student’s participation packet.

Student ID is required for submission. All Student IDs can be found in Family Access of Skyward on the Student Information tab on the right side of screen. The Student ID is under “Other ID.”

An annual physical exam completed on the UIL Physical Evaluation Form and signed by a doctor is required for all student athletes, cheerleaders, band, and drill team members. The UIL Preparticipation Medical History and Physical Evaluation form is located under the “Physical Form”. A HARD COPY OF THE UIL PREPARTICIPATION MEDICAL HISTORY AND PHYSICAL EVALUATION MUST BE TURNED IN TO HIGH SCHOOL ATHLETIC TRAINERS, MIDDLE SCHOOL ATHLETIC COORDINATORS, OR BAND DIRECTOR.

All other participation forms are located under “Electronic Participation Forms”. Please select the top form from the drop-down menu and proceed to complete all GCISD and UIL forms.

To sign each document, click inside the signature box and hold your mouse down. This will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box.

The documents can be printed once completed by the parent and student.

You will receive a confirmation email once GCISD staff has reviewed the document.
